Insurer Licence

The relevant application form, duly completed and accompanied by supporting documentation and the prescribed fee, is used when applying to carry on insurance business as a Class "A", “B”, “C”, or “D” Insurer.

Insurance Broker License

This form, fully completed and accompanied by the necessary supporting documents and the required fee, is used to apply for a license to operate as an insurance broker in or from the Cayman Islands.

Insurance Agency License

This form, when properly completed and submitted with the necessary supporting documents and required fee, is used to apply for a license to operate as an agent for an insurance company authorized to conduct business within the Cayman Islands.

Insurance Manager License

This application form, once fully completed and submitted with the required supporting documentation and fee, is used to apply for a license to operate as an insurance manager within the Cayman Islands.

Annual Return

According to the Insurance Law, all licensed Class ‘A’, ‘B’, ‘C’, and ‘D’ insurers, as well as insurance managers, agents, and brokers, are required to submit their annual returns along with financial statements. These filings must be completed within six months of the entity’s financial year-end and should be emailed to [email protected].

Affidavit

This is the standard affidavit form to be used when submitting director or shareholder applications.

Cyber Incident Report

This form is designed to provide a structured and standardized way of notifying the Authority about cyber breaches, and to submit periodic updates until the incident is resolved. A cyber incident should be reported immediately if it is deemed to have a material impact or has the potential to become a significant issue. Material cyber threats may include situations such as significant data loss or compromise of system controls; potential impact on a large number of clients; unauthorized access to your systems; introduction of unauthorized material, software, viruses, or spyware; and/or alteration or extraction of data from your systems. Personal Questionnaire

The Authority mandates that every director, officer, manager, and any shareholder who is a natural person holding more than 10% ownership, and who wishes to be appointed to a company licensed under the Insurance Act, must complete a personal questionnaire. This form should be submitted along with the supporting documentation outlined in the Insurance (Forms) Regulations.
