This form is designed to provide a structured and standardized method for notifying the Authority of cyber breaches and for submitting periodic updates until the incident is fully resolved. Any cyber incident that is considered to have a material impact or has the potential to escalate into a significant issue should be reported immediately. Material cyber threats can include significant data loss or system control failures, potential impacts on a large number of clients, unauthorized access to systems, introduction of harmful software, viruses, or spyware, and/or the alteration or extraction of data. The Authority mandates that all individuals who are directors, officers, managers, or shareholders of a company licensed under the Cooperative Societies Act or the Building Societies Act, complete a personal questionnaire. This questionnaire, along with the necessary supporting documentation, must be submitted in accordance with the licensing requirements.
